Fernandez, Yvette M.

Dr. Fernandez is board certified in both internal medicine and geriatrics. She sees patients in the office and via virtual visits. She serves as medical director at Pomperaug Woods Health Center and River Glen Healthcare Center, where she cares for rehabilitation and long-term patients. Dr. Fernandez graduated from Albert Einstein School of Medicine, completed an internal medicine residency at Montefiore Medical Center and then geriatric medicine fellowship at New York Medical College. She is dedicated to developing a close physician-patient relationship and supporting the elderly in achieving maximal independence.
