Garden Terrace – Centers of Alzheimer’s Excellence, a Life Care Center of America facility, is proud to provide rehab for individuals with Alzheimer’s, dementia and other cognitive impairments. This patient population requires an appreciation of all aspects of their lives and our specially trained team considers each patient’s likes, dislikes, prior living status, personal habits, family involvement and the nature of their cognitive impairment as part of the therapy plan, in order to better motivate and interact with dementia patients to achieve the best possible outcomes. Dementia rehabilitation is a focus at this facility, providing comprehensive individualized programs and services. The therapy team communicates closely with physicians, nurses and social services, dietary and activities personnel to provide comprehensive care. This facility uses an interdisciplinary team approach, which involves patients and families in establishing a plan of care with the goal of returning to home and community life.
