Welcome to Cornerstone Family Practice, PLC! We are proud to be recognized as a Medical Home by Blue Cross Blue Shield. A Medical Home is not a building- it is like a "home base" for your health care needs. It is a trusting partnership between a doctor-led health care team and an informed patient.

~ We provide care for all ages, from newborns to geriatrics. Our services include: Health & Wellness / Preventative health services, including well child and adolescent visits, yearly health maintenance visits, Pap Smears, Prostate exams,& skin cancer screenings.

~ Acute and Chronic Illnesses: We manage a wide range of chronic illnesses. We encourage our patients to schedule regular visits for chronic conditions. We also provide acute care, including same day visits and minor emergency care. We provide a wide range of skin care, including acne management, warts, lesion removals, and trigger point and joint injections.

~ We keep appointments open each day so that patients who call can be seen the same day.

~ Web Based Communication: Our Practice Portal gives us a 2-way, secure, encrypted way of communicating. Please call if you would like to be signed up with our patient portal. The web portal can be accessed only through an invite from our office, so please give us a call to get that started. We check this portal hourly for incoming messages. This is a quick, secure and easy way to ask a question, request an appointment, or ask our biller about billing details. Please do not use the e-mail for critical concerns, or acute symptoms.
