Avalon Hospice

At Curo Health Services, and its affiliate hospice services like Avalon Hospice, we view hospice care from the perspective of the patient, not the disease. We use a holistic, patient-centric approach. We evaluate the patient, the prognosis, the family situation and any other contributing factors in order to develop a Hospice Plan of Care. We also recognize the importance of the primary care physician’s relationship with the patient and family. We work closely with you to ensure your patients’ every need is addressed while they are in our care.

Our Interdisciplinary Team, which includes our Hospice Medical Director, will involve you in decisions made about your patient's treatment. We will rely on your knowledge of the patient's history and family situation to help guide us as we strive to provide the best care possible. You may be as involved in your patient's care with us as you wish to be.
