CHI Memorial Community Health - Hixson

CHI Memorial has worked for more than a decade extending its ministry to the underserved through CHI Memorial Community Health. Our center offers a wide range of services for teens and adults and is center is managed by a team of nurse practitioners through a collaborative agreement with a primary care physician. Services include primary health care, well-woman checks, including annual pelvic exams and pap tests, adult physicals, screenings, diagnosis and management of chronic diseases, laboratory services, X-ray services and patient education.
