Signature Health and Wellness

Signature Health & Wellness was founded in 2006 by Dr. Moses A. Swauncy to provide quality medical care for adults and children. We offer treatment and management of a variety of conditions such as Hypertension (high blood pressure), Diabetes, Thyroid Problems, Cholesterol, Heart Disease, Depression, Anxiety, ADHD, and other chronic conditions including Medical Weight Loss. We also perform annual physical exams, work physicals, school physicals, lab testing, and allergy testing and treatment. We also see acute issues that may arise like strep throat, upper respiratory infections, ear infections, urinary tract infections, sexually transmitted diseases, and a host of other medical problems.
Dr. Swauncy and the team at Signature Health & Wellness have been active in the Rutherford County community for over a decade. Participating in Old Timer’s Day in LaVergne, the Smyrna Christmas Parade, The LaVergne Senior Citizens Center, and Family Day at the Nissan Plant in Smyrna. Signature Health & Wellness is most proud of its relationship with Carpe Artista which is a non-profit organization in Smyrna, Tennessee, that teaches children music, drama, and visual arts. Dr. Swauncy serves on the board of directors with Carpe Artista. His passion for the arts began at a young age when he trained in classical piano and saxophone.
We are located in LaVergne, Tennessee, and we hope to continue to serve the families of North Rutherford County (LaVergne and Smyrna) as well as Antioch and Nashville. We look forward to providing you and your family with exceptional care for years to come!
