Ascent Healthcare is a post-acute care practice composed of physicians and advanced practice providers. We provide value-based, innovative, and compassionate medical services in the post-acute care arena through partnerships with skilled nursing, long-term care, assisted living, memory care, physician groups, health systems, and payors in the Pacific Northwest. Ascent’s providers enjoy the challenge and complexity of their diverse patient population and the flexibility in work schedule we offer.

Ascent Healthcare is not a free-standing clinic and, therefore, cannot accept new patients from the community. We only provide PCP coverage for patients residing in a Nursing Home, Assisted Living, or Memory care community.
