Gundersen St. Elizabeth's Care Center Shields Ave. is an 80-bed skilled nursing facility near our main campus. It includes a specialized Alzheimer's Unit for residents with moderate to severe dementia. Our compassionate caregivers provide holistic, person-centered care for residents that require a short-term rehabilitative stay or need long-term care. Together, we create an individualized care plan that includes medical and therapeutic care, meaningful activities, social connections and family engagement.
