Gold Star Hospice Care is on a mission to serve those who have served their country. America’s Veterans often carry experiences that present unique challenges at the end-of-life. We Honor Veterans®, a program of the National Hospice and Palliative Care Organization and National Hospitalization Avoidance Program, is designed to empower hospice professionals to meet the special needs of dying veterans.

Gold Star Hospice Care patient care and services are guided by Individual Plans of Care that are developed for our patients and their loved ones by our interdisciplinary team of physicians, nurses, social workers, chaplains, aides and volunteers.
