In consultation with the patient’s Primary Physician and the Hospice Medical Director, our Hospice team prepares an individual Plan of Care based on the patient and family’s needs and goals. Direct care 1s provided by the primary caregiver and is augmented by Our Hospice Team. Hospice care is provided wherever the patient calls home.
