I work to assist patients who have multiple chronic illnesses through assessment, counseling, care management, advocacy and linkage to long-term community and home-based services, and facilitation of continuity of care and coordination of services. I utilize a solution-focused approach along with psychoeducation and motivational interviewing to assist patients in meeting their stated physical and mental health goals. My clinical interests include integrated health, palliative care and chronic disease self-management.
