Heidi Hoffman, MS, CRNP

Heidi Hoffman, CRNP, is a nurse practitioner with AHN Primary Care Institute, specializing in adult medicine. Her clinical interests include internal medicine, cardiovascular disease and risk reduction, diabetes, disease prevention, geriatrics, hyperlipidemia, musculoskeletal pain, and preventive health and wellness.

Heidi received her Bachelor of Science degrees from Allegheny College in Meadville, Pennsylvania, and from University of Rochester in Rochester, New York. She received her Master of Science from University of Maryland in College Park, Maryland.

Heidi is an ANCC board-certified adult nurse practitioner. She has been in the community practice for 20 years and has valued the many moments she has shared with her patients’ and their families’ lives. She looks forward to being a part of the well-being of her community each day. She sees patients ages 18 and older.
