APC Home Health Services

APC Home Health Services is dedicated exclusively to providing home health services to Medicare and Medicaid eligible patients on a part-time and intermittent basis. Skilled nursing care, home care aides, physical therapy, and occupational therapy, are provided to homebound geriatric patients whose care is provided in compliance with the orders from the attending physicians. As our short-term Client, you’ll find we share your goals as you work toward recovery after illness, injury or hospitalization. You’ll experience a specialty team approach to providing personalized therapy and health care that helps you regain target levels of strength, mobility, independence and wellness.
