Ascent Care Partners

ACP partners with primary care practices to provide clinical management support to Medicare patients “in-between their office visits” who have two or more chronic medical conditions. The service ACP provides is referred to as Chronic Care Management (CCM) which is a Medicare benefit intended to also improve the coordination and management of patients’ overall health. Acting as a practical extension of physician practices, ACP employs a team of Care Coaches supervised by Nurse Practitioners to engage patients (using a variety of scripts and surveys). The monthly phone calls cover topics such as medication adherence and disease-specific management plans specific to each individual patient.
