John D Chase, MD

Dr. Chase was born in Cleveland, Ohio. He attended Northwestern University in Chicago for undergraduate studies and medical school, graduating with his MD in 1978. He did his Neurology internship and residency at the Baylor College of Medicine in Houston from 1978 to 1982. After finishing residency, he moved to Indianapolis joining a neurology practice 1982. In 2006, he opened a solo practice in Greenfield and New Castle. He joined American Health Network in 2011. He lives with his wife and two dogs. They have been married for 34 years. They have a son and three daughters. In his free time, he like to travel, hike in state and national parks and visit family. Hobbies include playing the piano, woodworking and bicycling.
