We are located in the Trinity Health IHA Medical Group, Primary Care - Brooklyn Medical Center on Main Street in Brooklyn.

If you have an urgent medical problem and need assistance when the office is closed, please call the office and your call will be directed to the Trinity IHA Call Center. The call center staff will assist you and contact the physician on-call to address your concerns.
