Center For Medication Management

Blanchard Valley Hospital offers an on-site outpatient pharmacy, Center for Medication Management, for your medication needs, 419.429.7002. Prescriptions can be filled at the Center for Medication Management, Monday - Friday, 9 a.m. - 6 p.m. located at 1900 South Main Street, Findlay, Ohio 45840. An automated, after-hours prescription refilling system can also be accessed by calling 419.429.7002.
The Center for Medication Management also provides complete disease management to assist patients with their health care. With an order from the patient's primary care provider, patients can work one-on-one with a clinician to develop a program based on their needs and condition. Patients will learn how medication affects recovery and have medication dosages monitored/adjusted in collaboration with their physician.
