Rainbow Hospice Care

Rainbow Hospice Care is a community-based non-profit organization that provides a full spectrum of in-home and inpatient care services for terminally ill patients and their families living in Jefferson County and in portions of the six surrounding counties in south-central Wisconsin.

Our mission is to provide comfort, care, and meaning throughout the end-of-life journey. Headquartered in Jefferson WI, with a state-of-the-art inpatient center in Johnson Creek, Rainbow Hospice Care was founded in 1990 and serves patients with any type of life-limiting illness and a life expectancy of six months or less. All patients and their loved ones are treated with respect, kindness, and concern—while focusing on quality of life and dignity with care that is specifically tailored to each individual’s unique needs and wishes.
