From your first visit, our therapists will determine the best plan of care based on your unique physical and functional needs. Your treatment plan will be individualized based on physician’s orders. Once the treatment plan is developed, regular updates and progress reports will be provided to your physician.

Planning your first visit: Please bring your physicians prescription for therapy, a picture ID, your insurance card(s) and a list of your prescribed medications. Wear comfortable clothes, for example a t-shirt and sweatpants. Please arrive 10-15 minutes before your scheduled appointment to fill out required paperwork in our office.
