Our post acute care rehabilitation programs are designed to transition patients from the hospital back to their regular lives safely, quickly and with confidence. And when it comes to care plans, one size never fits all. We respect every person’s individuality, and tailor care to their needs.

Five acres of rolling green grass. Our landscape is almost as healing as our post acute care. After a busy day, our residents and patients can relax in a full body whirlpool, dine restaurant-style (often with live music) or get a trim in the barber and beauty shop. Linger outside and tend to the garden, or head over to the beautiful Shawano Lake and Wolf River. Memory care residents and patients have their own secure community, indoors and out.
