Reach is an established home health agency. Our organization was developed in response to patients who needed assistance with health care and personal needs and wished to remain in an independent living environment. We provide a cost-effective alternative to skilled care by utilizing highly trained caregivers as an integral part of our healthcare delivery system.

Starting with a thorough evaluation by a Registered Nurse (RN) Case Manager, we determine the patient’s health status and medical/personal care needs. The RN will determine if the patient’s needs can be met by an unlicensed caregiver based on the stability of the patient’s status, the nature of the tasks, and the training of the caregiver. The RN will develop a plan of care for the patient including the frequency of supervision required.
