Our focus is no longer the illness our patients live with but the symptoms that interrupt their quality of life. Our Embrace team of nurses, doctors, social workers, volunteers and other care providers will partner with you to create a customized care plan that fits your unique diagnosis, environment and support system. Hospice care can be provided at home or in a healthcare facility.
